Athletic Physicals – Required to Participate
A current athletic physical dated on or after April 15, 2026 is required for your child to participate. If your child already submitted one for our Fall season, there’s no need to resubmit. If not, you can submit your child’s physical by uploading a scanned copy using our secure online portal: https://church.
